How to begin participating:
Before trying out, practicing or participating in any sport, all athletes are required to have a completed sports packet approved and on file. This includes a current sports physical performed by a physician, on the OSAA form. Sports physicals are good for two years.

Sports Packets are printed and available in the office.
Alternative options for sports physical examinations:
Roseburg Urgent Care
Office Hours: 8 a.m. to 8 p.m. M-F
Evergreen Urgent Care
Office Hours: 7 a.m. to 7 p.m. M-F; 9 a.m. to 5 p.m. Saturday; 11 a.m. to 5 p.m. Sunday
South River Urgent Care
Office Hours: 7 a.m. to 6 p.m.
All athletes must have health insurance coverage before they can participate in any sport at Jo Lane. This includes try-outs and practices.
If you do not have coverage, you will need to purchase it.
One option is to purchase student accident insurance online through, United Health Care.
If you do not have a computer or need assistance, contact the Athletics Office at 541-440-4105.
Sports packet instructions:
- Print the entire packet
- Take forms along to get physical (physicals must be completed on the proper form)
- Complete forms
- Sign all indicated fields
- Return the entire packet to the Athletics Office
- Allow time for processing before the first day of practice or try-outs
