Requirements

How to begin participating:

Before trying out, practicing or participating in any sport, all athletes are required to complete sports forms on Rank one CLICK HERE. A current sports physical performed by a physician, on the OSAA form is also required. Sports physicals are good for two years. Below are additional forms that will need to be turned into the athletic office.

Insurance

All athletes must have health insurance coverage before they can participate in any sport at Jo Lane. This includes try-outs and practices.

If you do not have coverage, you will need to purchase it.

One option is to purchase student accident insurance online through, United Health Care.

If you do not have a computer or need assistance, contact the Athletics Office at 541-440-4105.

Sports packet instructions:

  • Fill out of Rankone.com 
  • Take OSAA Physical forms along to get physical (physicals must be completed on the proper form)
  • Complete forms
  • Sign all indicated fields
  • Return all additional forms to the Athletics Office
  • Allow time for processing before the first day of practice or try-outs
